You can help us speed up your registration process. Just click here to download the required PDF form. Please fill out this three page form prior to your first office visit and bring it to our office on the day of your initial appointment with a copy of your insurance card and a photo ID or fax them to us at (808) 528-1719. This will allow us to serve you more efficiently and save you time. If possible please try to avoid wearing makeup or body lotions as this may interfere with the examination. We will address your primary complaints and questions regarding any necessary treatments or procedures but evaluation of additional dermatology problems may require an additional appointment. Some surgical procedures may require another appointment, they also may require pre-approval by your insurance plan.
Your scheduled appointment time has been reserved specifically for you. We understand that sometimes appointments must be changed due to inconvenience or illness. We request 24-hours notice if you need to cancel your appointment. Patients who miss their scheduled appointment without calling to cancel may be charged a fee that will not be covered by insurance.
